Health Risk Assessment

WakeWise Wellness would like to invite you to take a step toward a healthier you by taking part in Cigna's on-line Health Risk Assessment (HRA).  No matter which Cigna health insurance plan you signed up for; you and your spouse (if covered) can each have $50 added to your medical deductible accounts just for completing the assessment.  A Health Risk Assessment (HRA) is a questionnaire that asks about your current lifestyle and habits in various aspects of life, including health habits, physical activity, nutrition, injury prevention, and stress management. The confidential results of your HRA can be used to help set goals for healthy actions in your personal health plan and are provided to you immediately after you complete the assessment.

When you are filling out the Health Risk Assessment you'll see a place to put in some current Health Screening Information/”Know Your Numbers” such as –your last blood pressure; total cholesterol level; HDL cholesterol; LDL cholesterol; triglyceride level; your height and weight and waist circumference.  Please have this information handy when you fill out the Health Risk Assessment.   This information is usually available after completing an annual physical. We recommend that you see your doctor and have these tests and measurements done at your next scheduled physical this year.

Here is how you can participate if you have access to a computer:

1. Go to www.myCIGNA.com or click on the hotlink- mycigna logo on wakeemployees.com
2. Log in with your User ID and Password and select “Go”
3. If you have never registered- go to www.myCIGNA.com log in screen and select “Register” and complete the initial registration process.
4. Once logged in select “My Plans” tab at the top
5. Next page-select “I want to” and then “Take my health assessment”
6. Next page-select your name and a new window will open to the My Health and Wellness Center log-in page
7. On the log in page select “Not yet signed up?” so that you can register for a my health and wellness center account.
8. Follow the registration instructions and complete all required fields.
9. When registration is complete, the next page will be the my health and wellness center home page

A $50.00 incentive will be added toward your account once you have submitted your completed Health Risk Assessment Questionnaire.

To receive an additional $50.00 toward your deductible expense you must let us know that you have received the Health Screening Information from your doctor.  Go to wakeemployees.com and under WakeWise Wellness click on the link “Health Screening Form.” To complete this form you will need your name, CIGNA ID number, the date you received your health screening, and your doctor’s name.

Your participation is completely voluntary.  In addition to extra credit to your deductible, you will benefit from getting an accurate picture of your current health status and provide the health counselors at Cigna information to help furnish you with resources and assistance to help you make improvements in your health.
