Petty Cash & Change Fund

This form is used to set up new petty cash funds & change funds and also to make changes to existing funds. Please fill out all pertinent information for the type of fund or change you are requesting and send completed form to Karen Thiessen. If this is a new fund or if a change is being made to increase an existing fund, a check will be mailed to the fund custodian at their work address.
